BIRTH CERTIFICATES
Birth Certificates are available from the City Clerk's Office if
- - The birth occured in Boone County
- - The birth occured in 2010 or later
Who can request a Birth Certificate
- The individual named, if 18 years of age or older.
- A parent, if the parent's name appears on the Birth Certificate as mother or father.
- A Legal Guardian, with the Court Order stating said guardianship.
- The person's legal representative.
- A department of the State or Federal Government
- Any person upon order of a court
How can a person get a Birth Certificate?
- In Person, at the City Clerk's Office, 401 Whitney Blvd., Belvidere, IL 61008. Monday - Friday, 8:00am to 5:00pm.
- By submitting a request by mail. Certified copies will be mailed after request and payment have been received.
Any request for a certified copy of a vital record requires a copy of the applicant’s current/vaild photo ID per the Illinois Division of Vital Records. Please present a current/valid photo ID when applying in person, or attach a copy if applying via mail.
Cost of Certificates are as follows
- -$14.00 for the first copy
- -$6.00 for each additional copy
