DEATH CERTIFICATES
Death Certificates are available from the City Clerk's Office if
- - The death occurred in Boone County
- - Death occurred in 2001 or later
- - If Death occurred prior to 2001, please contact the County Clerk's office 815-544-3103
How can a person get a Death Certificate?
- In Person, at the City Clerk's Office, 401 Whitney Blvd., Belvidere, IL 61008. Monday - Friday, 8:00am to 5:00pm.
- By clicking here and completing the Application for Search of Death Records Files.
- Certified copies will be mailed after request and payment have been received.
Any request for a certified copy of a vital record requires a copy of the applicant’s current/vaild photo ID per the Illinois Division of Vital Records. Please present a current/valid photo ID when applying in person.
Cost of Certificates are as follows
- - $14.00 for the first copy
- - $6.00 for each additional copy
